The first thing you need to do is make sure you meet Todd Baird Lindsey Foundation requirements. Go to the Requirements tab for more information. If you think you meet our requirements, the second step is to obtain an application. You can obtain an application by doing one of the following:

After your completed application is received, our Executive Director, Stacie Nickel, will contact you to schedule an appointment. At the time of your scheduled appointment, Stacie will visit you in your home. Please have proof of your income, assets, all your bills, your picture I.D., and medical cards available at the time of the scheduled visit.
